Here are some tips for completing the intial intake information:

1. Under the section “Insurance Information”, only select “Yes” if you are intending on having us submit claims to your insurance provider, otherwise leave it as the default selection of “No”.

2. Under the section “Outcome Measures”, if you are NOT intending on using insurance, please leave the selection as the default “No Insurance” and move onto the next section. If you are intending on having us submit claims to your insurance company, then this section IS MANDATORY. Please select a category corresponding to your condition and complete a questionnaire for that category. Insurance will only require a single questionnaire, so only select more than one questionnaire if you feel it necessary.

3. When you finish the form, you must click the “SUBMIT” button in the top right corner, or we will not receive the information. If you click the “SUBMIT” button and you are not directed to another screen verifying successful submission, it is because you left a required section incomplete, so please complete all required fields and click “SUBMIT” again.
